Please bring these items with you on the day of your visit:

• Health Insurance Card/Information
• A friend or family member to take notes/help with questions
• A list of your questions /concerns
• A list of all medications you are taking
• A copy of your XRAYS / CT scans (actual films and reports)
• Copies of records from doctors related to your sinuses/nose/allergies/lungs.
• Completed FORM above.
